ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following. Other duties may be assigned.)
1. Provide support to the Registered Nursing Team by completing non-clinical tasks and administrative duties.
• Review and submit electronic MAR’s monthly.
• Update Consult Forms
• Update MD orders after appointments
2. Assist Residential teams in coordinating individuals’ Medical and Specialty Appointments
• Participate in Teams’ regularly scheduled Management meetings
• Confirm appointment details with clinics and teams for date/time, location of appointments – even those you are not attending
• Help prepare staff thoroughly for each appointment, past details, expectations, orders needed.
• Help train new staff in procedures/checklists for completing appointments successfully.
• Assist Teams in coordinating appointments: vehicle, day program needs, medications to be given at appointment.
3. Perform monthly inspections (Medication Cabinet PSR) of each residential facility’s medication and treatment cabinets/rooms for expired medications and all nursing supplies (gauze, Band-Aids, gloves, etc.) and replace expired items.
4. Maintain each residential facility with appropriate protective barrier equipment in compliance with the OSHA standard: biohazard sharps containers, waste containers, spill kits, CPR micro shields, and protective eyewear. Provide/maintain each residential vehicle with first aid kit and spill kit.
5. Assist management Teams in obtaining/refilling medications weekly and as needed; ensuring adequate amounts of OTC medications are stocked, working directly with pharmacies and management Teams.
6. Assure function of support equipment: oxygen tank, suction machine, nebulizer, etc.
7. Keep consultation forms up to date for pertinent diagnoses and medication profiles. Add diagnoses to Therap IDF. Purge medical charts as necessary.
8. Notify primary nurses with pertinent health or medical changes in a person’s health.
9. Act as a resource for residential staff regarding medical and healthcare concerns.
10. Assist the Training Department with required staff training: STAMP, CPR, CPI etc.
11. Assist in training of new staff in completing their OPWDD/AMAP Peer Observations at sites.
12. Serve as a member of a committee as required or requested.
13. This position requires at least a two-week (14 day) notice for voluntary resignation of employment.
SUPERVISORY RESPONSIBILITIES
This position has no direct supervisory responsibilities

EDUCATION AND/OR EXPERIENCE
High School Diploma, GED, Current licensure in New York as an LPN or CNA. A valid New York State driver’s license. One year’s experience as a DSP in an OPWDD regulated facility preferred. Candidate must be able to obtain and maintain AMAP certification with the agency.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instruction, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively and deliver instructions or teach a skill.
MATHMATICAL SKILLS
Ability to apply basic concepts of basic algebra and geometry.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
PHYSICAL DEMANDS
This job requires working in varied work environments, the ability to drive, bend, sit, stand, walk and lift 50 lbs.
WORK ENVIRONMENT
The work environment is varied, a great deal of time is spent in residential homes, driving, and at doctor appointments with individuals. A flexible schedule is needed.
The rate of pay for this position is $41,600 annually, based on $20/hour
