ESSENTIAL DUTIES AND RESPONSIBILITIES

- Ensure the program is providing services to adequately meet the physical, social and developmental needs of people enrolled, as identified in their plan, and that adequate protection of person's health, dignity, safety, comfort, wellbeing, and civil, human and legal rights, and that their preferences are considered.

- Develop community resources to be utilized by the program participant's; inclusion in the community is the primary focus of day habilitation.

- Work with each person and his/her service team to ensure that individualized services are provided.

- Ensure records are developed and maintained in accordance with applicable regulations and best management practices, including ensuring that an annual day habilitation plan is developed and reviewed.

- Ensure that the program site is operated in accordance with agency policies, mission, values, as well as regulations governing the operation of day habilitation programs as prescribed by OPWDD.

- Provide supervision to all habilitation assistants and oversee personalized individual activity schedules. Provide direct services to enrolled individuals as needed.

- Interview, hire, train, evaluate, and discipline staff as needed. Maintain up-to-date personnel records including a chronological record of staff training, and an annual evaluation that provides an accurate description of the performance of each staff person.

- Monitor staff attendance records and process staff requests for time off and timesheets weekly.

- Ensure ongoing program development and communication by facilitating staff meetings, professional development meetings, and interdisciplinary treatment team meetings.

- Maintain a clean, organized, and stimulating environment that includes supplies that are appropriate for individual needs, age appropriate, in proper working order, and chosen by those who are enrolled.

- Monitor program finances and work with the Adult Day Services Director to develop an annual budget.

- Keep the Adult Day Services Director informed about the operations of the program site, including personnel issues, concerns involving people enrolled, and any other issues that may arise.

- Participate in meetings and in-service training as required by the Adult Day Services Director and agency policies.

- Maintain positive relationships and collaboration with other Agency programs, community agencies and businesses.

- Adhere to all Agency policies, mission, and values, with a particular emphasis on providing the supports desired by each individual to achieve the life he/she wants.

- Ensure that program is operated based on current "best practice" models and continually strive to make the program the best available.

- This position requires a three-week notice of resignation.

SUPERVISORY RESPONSIBILITIES

- Act as a role model for staff and demonstrate an understanding of the agency's mission.
- Ensure ongoing employee development and communication by facilitating staff meetings, staff training and providing a welcoming environment.
- Provide guidance and direction to the Habilitation Assistants in implementing the methodologies and strategies of the assigned individuals' Service Plan.
- Provide supervision/guidance to Habilitation Assistants in their activities with program participants.
- Develop schedules and give work assignments to staff (community outings, staff breaks, lunch coverage etc.). Notify supervisor of staffing needs to arrange and coordinate "on-call" staff to meet the needs of the individuals. Assist in scheduling on-call to work at the program.
- Notify supervisor and attend to staff complaints, concerns, or training needs.
- Attend interviewing of staff.
- Prepare, train, and conduct fire drills as assigned.
- Review staff documentation, training objectives, behavior data sheets, physical restraint sheets, staff accident forms for accuracy and completeness

QUALIFICATIONS
Education and/or Experience - BA/BS degree preferred and at least one year of supervisory experience. Experience working in an OPWDD program or with individuals with disabilities preferred.

Mathematical Skills - Ability to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage and to draw and interpret bar or line graphs.

Computer Skills - Must be knowledgeable about Internet software, Microsoft Office components including Word, Office, Excel, and PowerPoint. Becomes knowledgeable and trained in the use of Therap for plan writing, event reporting and ongoing communication.

Certifications, Licenses, Registrations - Must possess a valid NYS driver's license and an acceptable driving record. Successful completion of behavior intervention techniques, CPR/First Aid, Medication Administration Certification, and all other required training.

  • Physically able to bend/stoop, squat, push/pull, reach above/below shoulder level frequently.

WORK ENVIRONMENT

- Works with household cleaning solutions.
- Moderate noise; 75-90% of work is performed indoors, 10-25% outdoors.
- Works with wheelchairs, electric/hydraulic/manual lift/ramp equipment, lifts and hook-ups, oxygen tanks, household appliances, cleaning equipment, augmentative communication devices and office equipment.
- Wear proper Personal Protective Equipment at all times.
- Must clean up human waste and other body fluids as required.
- Responsibilities occasionally may require an adjusted work schedule including overtime

The rate of pay for this position is $47,008 annually, based on $22.60/hour
